Am I eligible for a Forteo patient assistance program?

To qualify for the Forteo patient assistance program, you must meet one of these criteria:
  • For Singles: You earn less than $40,000 annually 
  • For Couples: You have a combined annual income of less than $60,000

Teriparatide (brand name Forteo), is prescribed for postmenopausal women and some men diagnosed with osteoporosis. It is recommended for people at high risk of fractures who cannot use other osteoporosis treatments.
